Pouchitis. The strongest evidence for the use of probiotics in IBD is in prevention and treatment of pouchitis (Table 5) [Mimura et al. 2004; Kuisma et al. 2003; Gionchetti et al. 2000, 2003]. After proctocolectomy with ileal pouch–anal anastomosis, pouchitis or acute and chronic inflammation of the ileal reservoir is the most frequent long-term complication of this operation, occurring in up to 20% of patients at 1 year. Studies of the microflora in the pouch have revealed deficiency of Streptococcal species [Komanduri et al. 2007]. This has led to a number of prospective controlled clinical trials of a probiotic, VSL#3 for 9–12 months, in the prevention and treatment of pouchitis [Gionchetti et al. 2000, 2003; Mimura et al. 2004]. These studies show consistently a decrease in incidence and relapse of inflammatory response. One uncontrolled trial in patients with mild active pouchitis who were treated with VSL#3, showed a remission rate of 69% [Gionchetti et al. 2007]. In contrast, a single species of Lactobacillus GG failed to show efficacy in a 3-month trial [Kuisma et al. 2003].

If you're healthy and want to stay that way... Shoot for 50 to 100 billion CFUs of a combo of lactobcillus and bifidobacterium. Starting with a large amount of CFUs will either keep you riding high (by supporting immunity) or trigger small but yucky reactions, like gas or nausea. The National Yogurt Association, who created this seal of approval, requires all yogurt manufacturers to include it on products that meet the standard of having a sufficient number of probiotics. The probiotics in store-bought yogurt need to meet this standard in order to be shelf-stable and reach your body alive. But even then, this standard isn’t all that useful in practice. Experts think balancing your guy may help treat anxiety, because it promotes better communications between the two. Opt for lactobacillus plantarum and bifidobacterium bi-fi0dum, two strains for anxiety, per research. Try 10 billion CFUs for three to five days. If your tum feels fine, go up by 10 bill.
The National Yogurt Association (NYA) of the United States gives a Live & Active Cultures Seal to refrigerated yogurt products that contain 100 million cultures per gram, or frozen yogurt products that contain 10 million cultures per gram at the time of manufacture.[49] In 2002, the US Food and Drug Administration (FDA) and World Health Organization recommended that "the minimum viable numbers of each probiotic strain at the end of the shelf-life" be reported on labeling,[50] but most companies that give a number report the viable cell count at the date of manufacture, a number that could be much higher than what exists at consumption.[51] Because of the variability in storage conditions and time before eating, it is difficult to tell exactly how many or how much active culture remains at the time of consumption.
Fifthly and finally, probiotics must be supplied in adequate numbers, which may be defined as the number able to trigger the targeted effect on the host.[citation needed] It depends on strain specificity, process, and matrix, as well as the targeted effect. Most of the reported benefits demonstrated with the traditional probiotics have been observed after ingestion of a concentration around 107 to 108 probiotic cells per gram, with a serving size around 100 to 200 mg per day.[18][not in citation given]

Can you take one probiotic for digestive health and a different one for vaginal health? “Unless recommended otherwise by a healthcare professional, I would recommend staggering intake within a day,” says Anthony Thomas, PhD, director of scientific affairs at Jarrow Formulas. In other words, take one probiotic with breakfast and the other at lunchtime.
This probiotic doesn’t contain any bacteria that produce histamine, a substance linked to inflammation, according to Nour Zibdeh, MS, RDN, CLT, an integrative and functional dietitian in Washington, DC. “Histamine intolerance is more common in women because of estrogen. The higher your estrogen level, the more histamine your body will produce. Symptoms of histamine intolerance include headaches, itchy and red skin, anxiety, congestion, post-nasal drip, stomach pain, diarrhea, and period cramps. If these symptoms are worse at ovulation or right before your period, I suspect histamine intolerance and recommend a probiotic that will help break histamine down.” Dr. Zibdeh recommends this brand. “It’s a mix of strains that are either neutral or may help metabolize excess histamine,” she explains.
What's more, people who have serious heart issues commonly have S. mutans in their heart valves—this is an undesirable type of bacteria that’s actually more often found in the mouth. If your oral microbiome is in balance, S. mutans are normally kept in control by more beneficial species, but if things get out of balance, they can reproduce and make their way into your bloodstream via openings in your gums, compromising your cardiovascular function.3
Unless you know that your body is lacking in a particular type of probiotic, you should just look for broad-spectrum probiotics that contain a mix of different strains of bacteria, Warren says: "We have billions of bacteria in our gut, so by taking a supplement with a range of different strains, you will ensure you are not overdoing or missing one type." Keatley also stresses the importance of finding a supplement with a diversity of bacteria strains to keep overgrowth of any one strain in check. "Providing too much of an advantage to one strain of probiotic may push out another strain we didn't know was important until it's too late," she points out.
Before buying this, I didn't realize that some strains of bacteria used in probiotics produce histamines. When I discovered this, I looked up which ones do, and all three I discovered are used in this one. If you're sensitive to histamines or have leaky gut (which makes you sensitive to histamines), then this is not for you. According to an article on bullet proof, an overload of histamines "leads to increased inflammation and many other symptoms including: skin irritation, hives, throat tightening, increased heart rate, nasal congestion, migraines, fatigue, heartburn, reflux, and weight gain." I'm sure this can work for some people, but probably only despite the histamine-producing bacteria.

I didn’t know there was much of a difference between probiotics until I did some research. Probably the first person of Western medicine to publish on the topic of probiotics in the early 20th century was the Russian Nobel Prize winner Ilya Metchnikoff, when he described longevity in people in Eastern Europe who lived largely on milk fermented by LAB. He theorized that proteolytic microbes in the colon produced toxic substances responsible for the aging process and proposed that consumption of fermented milk would coat the colon with LABs, decreasing intestinal pH, suppressing proteolytic bacteria and thus leading to slowing of the aging process [Gordon, 2008]. Until recently, testing of probiotics by ConsumerLab.com found that some products had far lower amounts of live organisms than claimed on the labels. But in 2018, it found that 17 out of 18 products tested contained the amounts listed; none exceeded limits for heavy metal contaminants. In contrast, in 2016, a study in Pediatric Research found that only one out of 16 Bifidobacterium longum supplements tested contained the species named on the label.
Your gut is diverse, so your probiotic should be too. Look for a supplement that contains multiple strains, sometimes listed as a proprietary blend. Inferior brands might only contain one probiotic strain such as Lactobacillus acidophilus. Aside from Lactobacillus and Bifidobacterium strains, probiotics may contain Strep. thermophilus and Saccharomyces boulardii, among others.

But with research into the microbiome still in its infancy, increasing evidence suggests that both commercial and clinical use of probiotics is outpacing the science.
In the small, new study, the researchers isolated 10 bacterial strains — five species of Lactobacillus bacteria, and five species of Enterococcus — in samples from 34 babies, identifying the strains as good candidates for crafting a probiotic cocktail of microbes that could survive in a human host's gut and stimulate SCFA production, according to the study.
