Fecal microbiota transplants (FMT), or "poop transplants," can treat a type of gut disorder with an infusion of diverse bacteria from a healthy digestive system, distilled from a donor's poop. This helps to correct imbalances of microbial diversity when the gut microbiome is dominated by the bacteria Clostridium difficile (C. diff), which can lead to serious gut disorders. 
Bifidobacteria were first isolated from a breast-fed infant by Henry Tissier, who also worked at the Pasteur Institute. The isolated bacterium named Bacillus bifidus communis[56] was later renamed to the genus Bifidobacterium. Tissier found that bifidobacteria are dominant in the gut microbiota of breast-fed babies and he observed clinical benefits from treating diarrhea in infants with bifidobacteria.
Because of the way that beneficial probiotic bacteria affects the digestive system, some researchers speculate that taking probiotic supplements could help to address symptoms of certain medical conditions. A clinical review published in "Canadian Family Physician" concluded that there was promising evidence that probiotics could be beneficial for people with a digestive disease called inflammatory colitis. They may also help to improve symptoms of vaginal infections. However, the review stated that more research was needed to prove the benefits of good bacteria for these conditions. Research is also investigating the possible benefits for those with irritable bowel syndrome. Some scientists also speculate that probiotics could be beneficial for travel-related diarrhea and other types of stomach upset. These proposed uses are currently under investigation.
As food products or dietary supplements, probiotics are under preliminary research to evaluate if they provide any effect on health.[3][71] In all cases proposed as health claims to the European Food Safety Authority, the scientific evidence remains insufficient to prove a cause-and-effect relationship between consumption of probiotic products and any health benefit.[3][44] There is no scientific basis for extrapolating an effect from a tested strain to an untested strain.[72][73] Improved health through gut flora modulation appears to be directly related to long-term dietary changes.[74] According to the National Center for Complementary and Integrative Health: "Although some probiotics have shown promise in research studies, strong scientific evidence to support specific uses of probiotics for most health conditions is lacking."[1]

Researchers found that probiotics can shorten diarrhea attacks in children. But they don't seem to work as well to prevent it. What are the benefits of taking probiotics? In the past five years, for example, several combined analyses of dozens of studies have concluded that probiotics may help prevent some common side effects of treatment with antibiotics. Whenever physicians prescribe these medications, they know they stand a good chance of annihilating entire communities of beneficial bacteria in the intestine, along with whatever problem-causing microbes they are trying to dispel. Normally the body just needs to grab a few bacteria from the environment to reestablish a healthy microbiome. Antibiotic-associated diarrhea (AAD) may be prevented by coadministration of probiotics, as suggested by several randomized controlled trials (RCTs). Several comprehensive meta-analyses, recently published, all show that probiotics significantly decreased incidence of AAD (RR 0.39–0.43) [McFarland, 2006; Szajewska and Mrukowicz, 2005; Cremonini et al. 2002; D’Souza et al. 2002]. The effects were similar across all categories and formulations of probiotics and treatment durations. The most commonly used probiotics were S. boulardii, LABs, and several combinations of LABs, given in doses from 107 to 1011, for durations of 5–49 days, generally paralleling the duration of antibiotic therapy. One of the meta-analyses found that S. boulardii, L. rhamnosus, and multiple mixtures of two different probiotics were the most protective against AAD [McFarland, 2006]. Other specific preparations have been studied to a lesser extent and that may be why their efficacy has been found to be less significant. One randomized, double-blind, placebo-controlled trial in individuals over the age of 50 using combination L. casei, L. bulgaricus, and S. thermophilus twice daily during a course of antibiotics and for 1 week after the completion of antibiotic therapy showed reduction in the incidence of AAD [Hickson et al. 2007].

Eating foods rich in good bacteria and using probiotic supplements may help provide protection from inflammatory bowel diseases, including ulcerative colitis and Crohn’s disease. The evidence is stronger, however, for an improvement in ulcerative colitis, while Crohn’s disease may not benefit as greatly. In addition, there is ongoing research studying the role of probiotics in gluten issues, including celiac disease.
Tempeh is an Indonesian probiotic food that is made from fermented soybeans. It is a high protein food that has a smoky, nutty flavor and is more firm than tofu. It tastes similar to mushrooms. Tempeh comes in patty form and is used by many people as a meat substitute. Soy tempeh is rich in a probiotic microorganism called Bifidobacterium bifidum. B. bifidum is a bacterium that may be particularly beneficial for those suffering from diarrhea, abdominal pain, and constipation. B. Bifidum helps boost immunity in the gastrointestinal tract. Bifidobacterium bifidum is also normally found in the vagina. Bacillus subtilis is another probiotic strain found in tempeh. Tempeh contains less calcium than milk, but the calcium in tempeh is very bioavailable, meaning it is very easy to absorb. Some bacteria used to produce tempeh manufacture vitamin B12.

Yet that’s how probiotics are often offered. They can be distributed in the United States as food, supplements or drugs. The regulations for each are very different. Most people looking for probiotics don’t see the distinctions. Ideally, the ways in which we use and consume probiotics would conform to the data and evidence that back them up. That rarely happens.

Two large-scale clinical trials recently reported in the New England Journal of Medicine suggest that the situation in infectious diarrhoea might also be more complex than previously believed. Freedman and colleagues did a randomised controlled trial of a probiotic containing Lactobacillus rhamnosus and Lactobacillus helveticus in children presenting to the emergency department with gastroenteritis. Contrary to expectations, they found that the probiotic did not prevent development of moderate-to-severe gastroenteritis within 14 days after enrolment. In a separate study, Schnadower and colleagues found similar results with L rhamnosus GG alone. Both trials used probiotics that are available over the counter in North America and showed no significant difference from placebo in the duration of diarrhoea and vomiting, number of unscheduled health-care visits, or length of absence from day care. These results cannot be generalised to other probiotic strains or preparations, but they do show that we have some way to go in elucidating which probiotics might provide benefits in which clinical settings.

Like kefir, kombucha is a fermented drink that may improve the function of the immune system and boost energy levels and detoxification. Beneficial bacteria in kombucha may improve antioxidant activity in the body as well. Kombucha tea comes in black and green varieties. Kombucha tea is made using a SCOBY, or a symbiotic colony of bacteria and yeast, that is necessary for the fermentation process. Saccharomyces boulardii is a probiotic yeast strain that is found in kombucha.

Stability and organism types — Some probiotic strains need to be kept cold in order to preserve their potency. This applies to their production, transport, storage and sales. Others are shelf-stable and don’t require refrigeration. Unfortunately, most refrigerated probiotics never make it past the stomach because they aren’t stable. Instead, look for a shelf-stable product that contains soil-based organisms.
The large intestine is home to hundreds of trillions of bacteria. Fortunately, most are neutral or even beneficial, performing many vital body functions. For example, they help keep “bad” bacteria at bay, play a role in immunity, help us digest food and absorb nutrients and may even have anticancer effects. But will consuming them as probiotics in foods or capsules make a notable difference to your health—especially if you are already healthy? Here’s a look at the evidence.
Prebiotics are non-digestible food components that promote the growth of healthful bacteria in your gut. In essence, they are "good" bacteria promoters. In addition to improving digestive health, prebiotics can also enhance calcium absorption. Prebiotics are components of nondigestible fibers and are found in many plant foods. Rich sources of prebiotics include garlic, onion, leeks, asparagus, dandelion greens, and sunchokes. Prebiotics and probiotics work synergistically to optimize gut health. Hench, meals or products that combine these together have a symbiotic effect.
Keep in mind that when supplements contain a specific number of organisms, this number may not be what is actually within each capsule at the time of purchase. Probiotics are living organisms and can die out easily. Especially if that supplement sits on your drugstore or warehouse shelf for months or longer, the number of organisms you get may be far less than what the bottle claims. Hardier strains have a longer shelf life. Capsule strength decays faster if the probiotic has been sitting around at elevated temperatures during transport to the store. Larger, longer and better studies are needed to test specific strains for specific conditions and to determine the proper doses and regimens.
