As for probiotics, fermented foods such as sauerkraut, kimchi, kombucha, kefir, and yogurt are naturally rich sources of live and active cultures (as well as digestive enzymes, which may be equally important for normal digestion). “Humans have eaten lots of different kinds of fermented foods throughout our history, for many thousands of years,” says Dr. Rawls. “That’s where the original idea for probiotic supplements came from.”
Prebiotics and supplementary ingredients — For probiotic bacteria to grow, they also need prebiotics. High-quality probiotic supplements have both prebiotics and other ingredients designed to support digestion and immunity. Two dangerous diseases in newborns, necrotizing enterocolitis (NEC) and neonatal sepsis, may meet their match with well-designed probiotic supplements. Both of these conditions are common in premature babies and are most dangerous in low birth weight and very low birth weight infants. Research has confirmed that when a pregnant mother takes high-quality probiotics during pregnancy, her baby is significantly less likely to develop either NEC or sepsis, particularly when the baby is breastfed after birth (and mom is still taking the supplements) and/or when probiotics are added to formula. To boost the immune system, B. Lactis is a promising choice. One study had participants taking either a probiotic or a placebo for six weeks. At the end of this period, researchers measured antibody levels and found greater increases in antibodies of the B. lactis group than in placebo participants, concluding that this probiotic may help improve immune function [1]. In addition, a 2009 study found that supplementation of the strain B. lactis DN-173 led to self-reported improvements in digestive comfort [2].
Sure, why the hell not? Lebwohl says: "Essentially a yeast infection is an overgrowth of a kind of fungus. In theory, a probiotic could potentially have an effect of the microbiome of the vagina, though proof of its effectiveness hasn’t really been established.” No harm in giving it a try as long as you don't succumb to the temptation to put yogurt in your vagina.
They then tested different doses of the 10-bacterial probiotic blend in mice, as well as in a slurry of human feces meant to mimic the environment of a human digestive system. The scientists found that even single doses maintained the healthy microbial balance and increased SCFA production in both the mice and the feces medium, the researchers reported.

In the past five years, for example, several combined analyses of dozens of studies have concluded that probiotics may help prevent some common side effects of treatment with antibiotics. Whenever physicians prescribe these medications, they know they stand a good chance of annihilating entire communities of beneficial bacteria in the intestine, along with whatever problem-causing microbes they are trying to dispel. Last year Jens Walter of the University of Alberta and his colleagues published a study that gives a glimpse of this potential future. The researchers decided to see what it would take to get the bacteria in a probiotic to successfully colonize the intestines of 23 volunteers. They chose a particular strain of Bifidobacterium longum that earlier studies had indicated could survive in the human intestine. In the study, the volunteers consumed either a drink containing 10 billion live B. longum bacteria or a placebo in the form of a glucose-based food additive (maltodextrin) each day for two weeks. Periodic fecal samples revealed higher than typical levels of B. longum in participants who did not consume the placebo.

Probiotics are thought to be akin to (and to increase the level of) the "good" bacteria found in your intestines. These "good" bacteria are thought to enhance our health through their support of our immune systems.

Prebiotics are considered by some to be non-digestible carbohydrates, that are not digested by the body but nourish the micro-organisms in the colon. They occur naturally in the diet and are found in foods such as garlic, bananas, oats, onions and leeks. This idea has been criticised by some due to its poor definition and some scientists prefer to use the term 'microbiota accessible carbohydrates', as they are fermentable dietary fibre that the microbes can use. However, foods containing prebiotics are also the components of a healthy diet and should therefore be consumed regularly.

Tempeh is an Indonesian probiotic food that is made from fermented soybeans. It is a high protein food that has a smoky, nutty flavor and is more firm than tofu. It tastes similar to mushrooms. Tempeh comes in patty form and is used by many people as a meat substitute. Soy tempeh is rich in a probiotic microorganism called Bifidobacterium bifidum. B. bifidum is a bacterium that may be particularly beneficial for those suffering from diarrhea, abdominal pain, and constipation. B. Bifidum helps boost immunity in the gastrointestinal tract. Bifidobacterium bifidum is also normally found in the vagina. Bacillus subtilis is another probiotic strain found in tempeh. Tempeh contains less calcium than milk, but the calcium in tempeh is very bioavailable, meaning it is very easy to absorb. Some bacteria used to produce tempeh manufacture vitamin B12.
When I polled friends and family about their personal experiences with probiotics, I got a real mixed bag of responses. Some called them a lifesaver and others felt no effects whatsoever. I put the anecdotes aside and talked to Ben Lebwohl, gastroenterologist and director of clinical research at Columbia University’s Celiac Disease Center, to get the straight poop on whether probiotics are miracle or myth.
Several large-scale studies and two meta-analyses have confirmed that probiotics should be a major consideration in determining natural remedies for diabetes. In a massive study involving almost 200,000 subjects and a total of 15,156 cases of type 2 diabetes, researchers confirmed that a higher intake of probiotic-rich yogurt reduced the risk of developing diabetes.

Unless you know that your body is lacking in a particular type of probiotic, you should just look for broad-spectrum probiotics that contain a mix of different strains of bacteria, Warren says: "We have billions of bacteria in our gut, so by taking a supplement with a range of different strains, you will ensure you are not overdoing or missing one type." Keatley also stresses the importance of finding a supplement with a diversity of bacteria strains to keep overgrowth of any one strain in check. "Providing too much of an advantage to one strain of probiotic may push out another strain we didn't know was important until it's too late," she points out.

However, it’s not just digestive woes that probiotics can help address. A clinical case series followed 300 patients who took a probiotic mixture of L. acidophilus and L. bulgaricus. They documented that 80% of acne patients had some degree of clinical improvement, particularly effective in inflammatory acne. Later, an Italian study involving 40 patients found L. acidophilus and B. bifidum supplementation produced better clinical outcomes in acne as well as better tolerance and compliance with antibiotics [2].
Given that you can get probiotics from the food you eat, you don't necessarily need to take a supplement, and Keatley says she prefers food sources of probiotics to supplements. However, she adds that "There are times when you really need a boost." One example might be during or after a course of antibiotics, as long as you have your doctor's OK, since antibiotics kill both good and bad bacteria: "Seeding your gut with good bacteria can lay the groundwork for a faster recovery and less constipation and diarrhea," Keatley points out. Angelone echoes Keatley's emphasis on food sources of probiotics, but also says that supplements can also play a useful role in "maintaining a healthy gut bacteria colony."
Previous studies have been contradictory, with some yielding more positive results about the benefits of probiotics, but most of them looked at probiotics in stool samples, not directly in the gut itself. In the new research, the scientists used more invasive methods to take samples of gut bacteria directly from different areas of the digestive system.
